The word 'face off' originally has nothing to do with plastic surgery. As a verb, 'face off' means to confront. Meanwhile, as a noun, it means a face-to-face duel or confrontation. In sport, it could mean as a starting point (usually in hockey) when two players from each team face against each other.
However, lately such word is related to beautification or face surgery. It means the facial operation which may lead to so little or much alteration to the original face of the patient that new face brings completely new look while the old face is taken away or 'off'.
